Customer Guidelines & Policies
Welcome to IC CARE where your health is an important issue for all of us. For over 20 years we have being providing “Health and Safety for Life.” At IC CARE we are happy to customize a radiation badge service to fit the needs of your office.
Badge Program:
- With your first shipment you will receive a badge and holder for each subscription you have ordered.
- ALWAYS Keep the holder. You do not need to return it every month or quarter, unless you are cancelling service.
- The badge is worn on the chest close to your heart.
- If you are pregnant, the Fetal badge should be worn on your tummy.
- On the TLD badge you will find the following information:

- Every month or quarterly (depending on your subscription) a new badge is sent out before the badge you are wearing expires.
- When you receive your new badge shipment, your old badges should be immediately sent back in the envelope provided no matter if the badge was used or not.
- Because badges are leased and re-used to keep your costs down, you will be accessed a Non-Returned Badge Fee of $20.00 per badge not returned to us within 15 days.
- If your new badge is late, continue to wear your existing badge until the new one arrives.
- Contact IC Care if you have not received your badge by the 10th day of the new period.
- Depending on mail delivery, badges arrive a few days to a week before or after the expiration date on your badge.
- With each order you will receive a Quick Start Guide where you will find instructions on loading the badge, how to send your badges back for exposure readings (reports), and a Postage Rate guide to help you determine postage needed to return badges. On the back of the Guide you will find a Change Request Form where you can make any changes to your account.
- Also, for changes on your account, we also provide email service for your convenience: changes@iccare.net
- If you return your badge on time, along with your next badge shipment you should receive your exposure report.
- As soon as you receive your new badge, follow the same instructions to load it.
- Immediatly return last periods badge to IC Care so your exposure reports can be processed in a timely fashion.
- Also, you may request a copy of your report by e-mail to myreports@iccare.net and of course we are here to help you by phone at 877 477-5486,and
and fax 877-477-5186.
- Also, with your new badge we provide you with a return envelope. DO NOT use a regular envelope as badges may be damaged or lost.
- Do not rubber band your badges together. Try and keep the envelope as flat as possible to minimize postage required by taping multiple badges
flat.
- Affix proper postage. Postage Rates on our return envelope:

Badge Instructions
- Badges will only slide into the holder in one direction. Please do not force.
- To insert badge, grasp the holder between your fingers with the clear plastic window facing up towards you.
- Slide the badge with the label facing up towards you into the open slot in the left hand corner. Slide the badge until it reaches the end of the holder.
- At the end of your wear period remove the badge from the holder and return for processing. Keep the holder! A new badge will be sent to you each wear period.
Policy
All badges are the property of ICCARE. The service you have purchased includes the use of a leased badge. The price you have paid for service includes the lease of the badge, reports, and customer services.
- There is no set-up fee
- There are not any charges for making changes on the accounts or processing fees for routine customer service
- Do not return holder, unless cancelling service. A lost holder will cost $7.50 to replace
- We do not charge extra for the exposure report; it is included with your service. Reports are available electronically ;[30 days] about 15 days after we receive your badge. Contact IC Care at myreports@iccare.net, or call us at 877-477-5486.
- There is a $20.00 fee charged for each unreturned badge.
- If you cancel your account you still need to return badges. Non-returned or damaged badges have a $20.00 fee per each standard badge and $50.00 for the RAD-Badge and you will receive a seperate invoice from IC Care.
- You should mail back the used badge as soon as your new one arrives. The only fee you will ever pay is for each badge that you do not return within 15 days. Badges need to be return for your Occupational Radiation Exposure Reports reading and Recycling. We are a green company!
- If you return your badge in a timely manner your exposure reports should be included with your next shipment.
- Your Occupational Radiation Exposure Report is Accredited by the “National Institute of Standards and Technology through NVLAP for the specific scope of accreditation under lab code 100555-0.”
Renewal and Cancellation Policy
For your convenience for uninterrupted service, IC Care will automatically renew your account each year unless you notify IC Care in writing 60 days before the renewal date.
Please notify IC Care in writing 60 days in advance if you would like to cancel your account.
Use the Change Request Form on-line for any additions, deletions, changes or cancellations as needed or you may Fax it to: 877-477-5186.
